If you refused the police department’s Intoxilyzer 5000 test, or another test of your blood or bodily substances, then you may have been given a Department of Public Safety 1205 form. This form constitutes a license suspension and may be dismissed if a responsive letter requesting a hearing is sent directly to the Department Driver Services within ten (10) business days! We can provide this letter for you.
